Does anyone know of a website that lists the requirements for COBRA Notification documents? I think our company's are sadly non-compliant and would like to redo them, but I don't want to leave out anything vital.

Hey Gert! just an aside, you should be getting templates for COBRA notification directly from your insurance provider, they'll be happy to help as a rule... and you do have over 20 employees to worry about? If not, COBRA is not federally mandated.

Thanks, Andrea, I hadn't thought of that. I work in a division of an organization of 1800 employees, so we are definately mandated!
