It's the duty of managers to "manage".  My take on this would be: write a memo to your own manager, attaching all the evidence, and either ask for his/her advice as to how to proceed or word your memo on the basis that you would like to raise your concerns, particularly as your colleague's dereliction means you cannot do your job effectively.  

On the other hand, sometimes you have to resort to guerrilla tactics, however unpleasant and unprofessional that may seem to you.  You may have to just "not manage" this situation and let the glitches become so obvious that someone very senior notices - and, hopefully, takes action.
