If so, how far do you go? Do you go to the nearby hotel (I have planned those in the past!) Have you done over night meetings? What type have you worked on?

Ours does occasionally though not often. Usually at a local hotel, but occasionally in other states. But it really is rare, as we have 100+ conference rooms on site. Usually it's an executive retreat

All the time. Sometimes it is just to get off site awsy from the norm, others are .. actual off-site in locations that require nite stays.
